Feedback Form Generator A feedback form allows you to get information from the visitors to your site. The visitors fill in a simple form and the information is automatically mailed to your account. All it takes are 4 easy steps to create your own feedback form. The form below will create the HTML code which you can then copy and paste into your homepage.
Enter the email address where you want the form information to be sent, the subject of the form and the URL where you want to send the visitor after he finishes the form.

Select the fields you want within your form. A field is like a label for the information you want Eg. Name, Age, Address, Email etc.

Note: You have two choices for field type viz. simple text field for small amounts of information and the 'TextBox' for large amounts of information.
